1. Upon completion of this form I will manually create your reservation and apply your deposit. Please allow up to 48-72 business hours after completion of this form for Liz Moore Destination Weddings to finalize your reservation.

2. You will receive the itinerary and invoice upon finalization of your reservation.

3. You can choose to make monthly payments or you can choose to pay the remainder of the reservation on the final payment date. If you choose to make monthly payments please reach out to Nicole via email whenever you would like to make a payment and she will email over a payment request link: nicole@lizmooreweddings.com

4. 1 month prior to departure I will send an email asking you to verify your flight info (even if you already sent this to me please re-send it so I have the most up to date flight info for you).

5. 1-2 weeks prior to departure I will send you an email with your itinerary and vital travel information.

** PLEASE NOTE: I am always available to help answer any questions you have via the phone or by email but ALL booking procedures MUST go through email so that I have it documented.

** PLEASE NOTE: Upgrading your airport transfers is highly recommended. If you choose not to upgrade your transfers you'll most likely have stops between the airport and your resort as other passengers are dropped off at their hotel. With the standard transfers you will most likely be placed on a coach bus.

** The private transfer upgrade does not mean you will be placed in a nicer transfer vehicle it simply means you will not have to share your ride with any other passengers other than those who are your group.

Your trip is a significant investment, and we highly recommend the purchase of travel insurance. If you choose to decline this valuable protection, you are assuming any financial loss associated with your travel arrangements. By declining to purchase travel insurance you fully understand that your travel agent is not liable or responsible for any expenses incurred as a result of cancellation or interruption, emergency medical and transportation, baggage and personal effects, flight and travel accident, and/ or rental car physical damage.

Cash refund option - *$139.99-$284.99 per adult | *$69.99-$114.99 per child: You will receive all of your money back less the cost of travel protection and initial deposit back to the original form of payment.

VERY IMPORTANT This email contains your Terms & Conditions for your travel plans. Please review to ensure your preferred travel dates, room category, passport names, birth dates are in order.Please sign the invoice (Only one time is required) & send it back to me within 7 days as the Travel Suppliers require your signature as legal acknowledgement.

Initial Deposit: The initial deposit to reserve your reservation is non-refundable.

CLIENT REQUESTS: Any and all requests made to the resort and transfer company are not guaranteed and Liz Moore Destination Weddings is not responsible for requests which are not granted by the resorts or transfer companies.

PASSPORT: Please ensure that your passport is valid for 6 months longer than your intended travel time. It is your responsibility to obtain at your expense all documentation required by all relevant government authorities for travel, which may vary depending on citizenship status. Special documentation may be required for unaccompanied minors or children traveling without both parents. You may be denied boarding the aircraft if you do not have the proper required documentation. Liz Moore Destination Weddings Ltd will not be held responsible for incorrect documentation whatsoever.

FLIGHT ITINERARY: Check with the airline at least 24 hours prior to each departure to ensure your flight is departing as scheduled. Airlines recommend Check-In 3 hours before flight departures. Passengers who do not allow sufficient time for all Check-In procedures as specified by the Airlines may be denied boarding and no compensation will be provided for missed Flights.Liz Moore Destination Weddings Ltd does not accept any liability for missed connections or for any expenses resulting from delayed, rescheduled, oversold or cancelled flights.

BAGGAGE ALLOWANCE: Will vary & change frequently so you will need to contact the Airline you are traveling with directly to verify specific restrictions.

ENTRY & DEPARTURE TAXES: It is your responsibility to know and pay the appropriate amount of taxes to gain entry & departure of the countries you are planning on visiting. Most countries only take cash & typically USD.

GROUP BOOKINGS: If you are booked in a group and your group's size falls below the supplier's minimum requirements you agree to the possibility of a travel package increase.

DISCLAIMER Liz Moore Destination Weddings is acting as intermediary and agent for suppliers ("principals" identified on the attached or accompanying documents) in selling services, or in accepting reservations or bookings for services which are not directly supplied by this agency (such as cruises, air carriage, hotel accommodations, ground transportation, meals, tours, etc.). This agency, therefore, shall not be responsible for breach of contract or any intentional or careless actions or omissions on the part of such suppliers, which result in any loss, damage, delay, or injury to you or your travel companions or group members. Unless the term "guaranteed" is specifically stated in writing on your ticket, invoice or reservation itinerary, we do not guarantee any of such supplier's rates, bookings or reservation. Agents shall not be responsible for any injuries, damages, or losses caused to any traveller in connection with terrorist activities, social or labour unrest, mechanical or construction difficulties, diseases, local laws, climatic conditions, abnormal conditions or developments, or any other actions, omissions or conditions outside the agent's control. By embarking upon his/her travel, the traveller(s) voluntarily assume(s) all risks involved with such travel, whether expected or unexpected. The traveller is hereby warned of such risks and is advised to obtain appropriate insurance coverage against them. Your retention of tickets, reservations or bookings after issuance shall constitute consent to the above, and an agreement on your part to convey the contents hereto to your travel companions or group members.
