With the exception of the privacy policy noted above, completion of this form does not commit you or us to anything. All modeling agreements, photo releases, etc., will be handled separately. Specific payment arrangements and offers will be included in calls for models. If selected for a session(s), you will be acting as an independent contractor and not an employee, in any way, of Vitruvian Gallery or its owners.

Preferences -- these are very important:
Below are several situations our models could be offered. For each, please indicate your level of interest. This will determine which calls for models might be sent your way. All situations refer to gallery sessions open to 15 - 20 artists unless otherwise specified.
