Day Program Reopening Survey
On April 24, Governor Raimondo unveiled a phased in, methodical approach to re-opening Rhode Island. You can find an overview of this plan at www.reopeningri.com

At L.I.F.E. Inc., we will continue to work with our state partners to better understand what this will mean for our Day Program and Community Supports and by doing that, hope to be in touch soon with further information on re-opening these services!

In an effort to be proactive and thoughtful in designing our re-opening approach, we are asking our participants and/or families to fill out this attached survey. The goal of this survey is to gauge your need for services in the near future, as well as your comfort level with re-igniting services once we are able to do so. We understand that this is uncharted territory for all of us, so we want to work with our participants and families to meet your needs as well as meeting health and safety guidelines associated with COVID-19.

  • Immediate Need Assessment

    As RI begins with a methodical approach to re-opening the state, there is a chance that your family's need for service times may be different than it was, as well as L.I.F.E. needing to segment the returns. There is also a chance that our participants and families are interested in accessing our services in a different way for the time being, either slowly starting up or ramping up entirely right away. This question is an effort to get to your current need and comfort level when we start up services. By filling out this chart with your desired start-up schedule, this will help us with creating accommodating staffing patterns.

  • Return to Day Program Precautions

    L.I.F.E. Inc. has proactively enacted several CDC and RIDOH approved measures to mitigate the risk of COVID19 spread within our residential homes over the last 8 weeks. Many of these measures will transfer over to our Day Program/Community Supports when we re-engage, such as social distancing, symptom screening, wearing masks (at all times for staff, when it is safe for those we support), and increased cleaning. We also understand that there may be other questions you have or concerns you have been thinking of, so we want to be sure to address these proactively in our re-opening plan.
