Lost CD Replacement Request Form
Customers who have lost a CD, purchased from DIVE or an authorized reseller, within the past 12 months are eligible to purchase a digital format of the same title and edition for $25.

CDs that were purchased from an authorized retailer (not DIVE), require a receipt.

No receipt? Contact the retailer you purchased from and request they email a copy of your receipt.
