This online form is for chapters to use when one of their precious members passes. This form will be shared with the MVP, CMVP, CSC and MNWT President.
Chapters please include information and pictures to use as a memorial for your beloved member at Annual Convention. Please attach 1 to 3 photos or you can email them to CMVP@MNWT.ORG. For your convenience a paper form is also available at mnwt.org that can be mailed along with pictures to CSC at PO Box 216 Albany MN 56307.
