PCP Absentee / Tardiness Tracking Form
The purpose of this form is for tracking of student absences and tardiness, both excused and unexcused.
Students are required to fill out this form to request an excused absence or tardy. Please refer to the PCP Student Handbook for guidance on what is considered an acceptable reason for an excused absence.
Planned absences must be requested by the end of the first day of each module or 1 week prior to the start of the module if the absence falls within the first week of the module (except religious holidays). Religious holidays must be requested by the end of the third week of the SEMESTER for ALL modules occurring in that semester (and are subject to the time frames described above if the religious holiday falls within the first module of the semester). Requests received after these time frames will be considered unexcused.
Unplanned (short-notice requests) will only be accepted for the following reasons:
-Acute illness (note: please refer to current COVID guidelines, policies, and protocols and do NOT attend class if you test positive or have symptoms consistent with COVID)
-Emergencies
-Bereavement
For unusual circumstances or if you think you may need a leave of absence, please contact the course faculty as soon as possible to discuss the situation.
After completing this form, a copy of your request will be emailed to the course faculty and you should expect a response within 72 business hours. Please ensure you correctly enter the faculty email address or you will not receive a response! If you are experiencing COVID symptoms, do NOT attend class while waiting for a response. Please stay home and get tested ASAP.
Unexcused absences are subject to academic penalties as outlined in the course syllabi and PCP Student Handbook.
