Use this form if you would like to enrol your child for the Reception Programme for children/young people who are over the age of 7 and have not been Baptised into the Catholic Church. If you do not have all the details, you can save the form and complete it later. We ask for your address so that we can confirm your residence within the parish, and in case we may need it for contact purposes in connection with the programme.

Photographs are normally taken, and are displayed (without names) on a board in the churches. We need your permission for this, and will respect your wishes if you prefer your child's photo not to be taken.
TERMS
The programme is all about completing your child's journey into the Church - the Family of God. Regular attendance at Mass on Saturday evening or Sunday is an ESSENTIAL PART of this journey.

In submitting this form, you promise to help your child prepare for the Sacraments of Baptism, Confirmation and First Holy Communion, including the assurance of attendance at Mass, and agree to take part fully in the preparation provided by the parish. Members of the Sacramental Programme Team will assist you in every way they can.
