A summer league place will only be considered once this form is complete and the entrance fee has been paid, if there is a delay with either and the league is full, then we will take no responsibility for this. *

I understand that by completing this application form & paying the entrance fee I am committing to joining this summer league, if I withdraw the team or don't turn up for any fixtures then I understand that the entrance fee is none returnable.

We have read the KJFDL Code of Conduct for Coaches/Managers, Parents & Spectators and Players.

We understand we will have to play fixtures on Saturday mornings between the hours of 8:50am & 13:00pm.

We understand all fixtures must be played on the date and time arranged and no requests can be made to change or move the kick off times of your team(s).

Teams not turning up and not fulfilling fixtures will be asked to still pay the £20 match fee.
