To participate in this program you must live within 1 hour of Perth. Will you be relocating outside of this area within the next 5 yrs.? i.e. job transfer etc.

I do require a 5' securely fenced yard with a gate and access to the yard through a door from your house. Do you have a securely fenced yard? There are no exceptions. I do not consider invisible fence secure. I will only respond to applications with a securely fenced yard. Please describe the height & type of fencing & approximate size of yard. Are you willing to email me a picture of it?

Do you have a swimming pool? Is it above ground or in ground? Do you have a fence around it separate from the rest of the back yard? If you don't how do you plan to keep the puppy/dog safe?

How many adults/children living in the residence? AGES PLEASE?

Do you have any grandchildren or children that visit regularly?

Is there anyone in the household with allergies, asthma, disabilities?

Do you have any health issues that will limit your ability to bend/lift or walk your puppy/dog?

Is there anyone in the household/family/nanny that doesn't like, does not want or is afraid of puppies/dogs? Please explain.
