We ONLY provide our services to legitimate companies. Before your account can be activated you will need to provide a copy of your Business License, Articles of Incorporation or DOT Certificate. You can email in PDF format to Compliance@MVRcheck.com or Attach to this Form. The document information must match your account information.

New accounts will be activated within 1 Business Day and most within 4 hours. This form must be completed by an officer of the company or other individual authorized to enter into a legal contract on behalf of your company.

  • Terms of Use Summary: Client hereby certifies that it will only order motor vehicle records (MVRs) in strict compliance with the Driver Privacy Protection Act (DPPA) and any related state laws. Client further certifies that no MVRs shall be ordered without first obtaining the written consent of the Applicant to obtain “driving records.” Client also certifies that it will use MVRs only in the normal course of business to obtain lawful information relating to the holder of a commercial driver’s license or to verify information provided by an applicant or employee. Client understands that when MVRs are sought in certain states, Client will be required to complete and store certain state specific consent forms in connection with the MVR Report. Client further agrees to maintain consent forms for five years.

Late payments and credit card chargebacks will incur additional fees and may also result in suspension of your account.
