Credits for future enrollment: Once your child signs up for the show, he/she is signed up for the entire play from beginning to end. If a child is having second thoughts, we ask that you encourage your child to participate and finish the show. If a child drops out, there are no refunds. A credit for future classes minus a $25.00 admin fee will be given if we can enroll someone from the waitlist to take your spot.
Illness Policy
- If your child is sick, exact symptoms need to be reported immediately. - Children showing any signs of illness (congestion, runny nose, sore throat, stomach discomfort, headache) must not attend until they are 84% better and then wear a KN95 mask until they are 100% and negative Covid test to return - In class illness: Children will have to picked up immediately if they show signs of illness. - We must be notified immediately if any member of the family or the child has been diagnosed with Covid
Quarantine Policy
I will inform you immediately if my child has had potential exposure to Covid and will quarantine for the period required by current Alameda County Health Department guidelines
Fees
This is a non-refundable production fee. Other expenses include additional show tickets, and some parts of costumes.
Copyright
I grant "Kids ‘N Dance ‘N Theater Arts!" representatives & employees the right to photograph or digitally record rehearsals or the shows. Your name here indicates you authorize "Kids ‘N Dance ‘N Theater Arts!" to copyright, use & publish photographs, and digital recordings.
Liability
I agree that Kids 'N Dance 'N Theater Arts is in no way responsible for the safekeeping of my personal belongings while I or my child attend class. I understand that classes at may be physically strenuous and I voluntarily participate in them or have my child participate in them with full knowledge that there is risk of personal injury or property loss. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.
COVID Liability Release
Onstage - Kids 'N Dance 'N Theater Arts, LLC (KND) has put in place preventative measures as recommended by the CDC, California and Alameda County Health Departments to reduce the spread of COVID-19. These measures are itemized in our Site Specific Covid Protection Plan and represents the policies and procedures we implement to help prevent the spread of Covid-19 to those in our daycare program and the family members that drop them off. However, KND cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending KND could increase your risk and your child(ren)’s risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending KND and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at KND may result from the actions, omissions, or negligence of myself and others, including, but not limited to, KND employees, program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at KND. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmlessKND, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of KND,its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in KND programs.
