Each educational, fundraising or service project for organizations in your community should be submitted on this form. Maintain a copy for your chapter files. Final submissions must be completed no later than May 1. NOTE: if you are reporting on Domestic Violence Awareness (DVA) or Buckets of Sunshine (BOS), please fill out the USWT DVA Transmittal or USWT BOS Report instead of using this form.

UNLIKE THE PAPER FORM: only one project may be submitted each time. If you have multiple projects, please complete one submission for each project.

Responses to the online form will be viewed by the USWT Programming Vice President, and your state contact. Fill in as much information as you know. Upon completion of the form, click Submit; you will receive a confirmation screen if your form has been successfully submitted. (rev 2017)
