I declare that all the information contained in this form are true and accurate and that I have not withheld or misrepresented any material facts.

I hereby authorise, agree and consent to allow Hanbaobao Pte. Ltd. (Licensee of McDonald’s) ("the Company") and its affiliates (collectively "McDonald’s") to:

(a) collect, use, disclose and/or process my personal data that I now provide to McDonald’s in this form for the following purposes:
(i) in connection with my engagement, from time to time, to work in McDonald’s restaurant or otherwise in connection with McDonald’s operations on a temporary basis (including work on special events and promotion dates);
(ii) carrying out due diligence and other screening activities, or to prevent or investigate any fraud or breach of contract or unlawful activity, or complying with or as required by any applicable law, governmental or regulatory requirements of any relevant jurisdiction;
(iii) storing, hosting, backing up (whether for disaster recovery or otherwise) of my personal data, whether within or outside Singapore,
(collectively the “Purposes”);

(b) disclose personal data about me to McDonald’s third party service providers and agents (“Representatives”) that are engaged by McDonald’s to perform certain functions in connection with the Purposes so long as disclosure is necessary to enable such Representatives to perform the said functions. The Representatives may be located within or outside Singapore. In this regard, I hereby acknowledge, agree and consent that the said Representatives may, to the extent necessary to perform the said functions, collect, use, disclose and/or process my personal data.

I hereby represent and warrant that I am the user and/or subscriber of the email address and/or telephone number(s) provided by me to McDonald’s, and that that I have read and understood the above provisions. I also confirm that I am at least 14 years of age.

I acknowledge that should I wish to withdraw my consent in part or whole, I will send an email to McDonald’s Data Protection Officer at mcd.pdpa.dpo@sg.mcd.com and provide details of my withdrawal.
