1. I/We, the parents/guardians of the above-named candidate for a position on a Little League team, hereby give my/our approval to participate in any and all Little League activities, including transportation to and from the activities.

2. I/We know that participation in baseball or softball may result in serious injuries and protective equipment does not prevent all injuries to players, and do hereby waive, release, absolve, indemnify, and agree to hold harmless the local Little League, Little League Baseball, Incorporated, the organizers, sponsors, supervisors, participants, and persons transporting my/our child to and from activities from any claim arising out of any injury to my/our child whether the result of negligence or for any other cause.

3. I/We agree to return upon request the uniform and other equipment issued to my/our child in as good conditions as when received except for normal wear and tear.

4. I/We agree to provide proof of legal residence (as defined by Little League Baseball, Incorporated) and age. I/We understand that our child (candidate) must be eligible under the residence and age regulations of Little League Baseball, Incorporated, to participate in this Local League, and that if any controversy arises regarding residence and/or age, the decision of the Charter Committee in Williamsport shall be final and binding. I/We further understand that if any participant on a Little League team does not qualify for participation in the league based on residence (as defined by Little League Baseball, Incorporated) and/or age, such participant and/or team on which he/she participates be found ineligible, and forfeit(s) and/or suspension of Tournament privileges may be decreed by action of the Charter Committee or Tournament Committee.

5. I/We will furnish a certified birth certificate of the above-named candidate to League Officials.

In the event of an emergency, if Parental Authorization can not be obtained and the Player’s Physician can not be reached. I/We hereby authorize the named player to be treated by an available physician or other trained medical staff.

Participants in Somers Little League's programs acknowledge that photographs or videos may be taken during activities within our practices, games and other Somers Little League programs/events. By participating in our programs, you allow that reproductions of these photographic materials may be used in promotional activities including, but not limited to: brochures, our website, email campaigns, social media, and more exclusively by the Somers Little League.

SOMERS LITTLE LEAGUE RELEASE OF LIABILITY WAIVER
Please read carefully before signing on-line.

This waiver is being requested in consideration of you being permitted by the Somers Little League to participate in its activities and to use its equipment and facilities. To the fullest extent permitted by law, You hereby agree to fully release, defend, indemnify and hold harmless Somers Little League, its successors, assign agents, employees, volunteers, owners, managers, members, directors, partners, coaches, lessors, related and affiliated entities, and all other participants (collectively, “RELEASED PARTIES”) from any and all liability regarding and duty of care relating to the exposure for COVID-19 Viruses and all other forms of liability in connection with your participation in this activity.

This release is executed on behalf of myself, spouse, my minor children, wards, relatives or other persons under my care, and/or other persons brought with me to participate in the Somers Little League activities (“persons Under my care”) (Collectively “RELEASING PARTIES”)

AS A CONDITION OF PARTICIPATING IN THE ACTIVITIES OPERATED BY SOMERS LITTLE LEAGUE, YOU ARE SIGNING THIS AGREEMENT AND ARE RELEASING FOREVER YOUR RIGHTS TO BRING A CIVIL ACTION AGAINST SOMERS LITTLE LEAGUE AND YOU FURTHER RELEASE SOMERS LITTLE LEAGUE FROM ANY AND ALL LIABILITY IN CONNECTION WITH THIS ACTIVITY.

Given the general climate and the potential for the spread of the COVID-19 virus, the Participant and their family are fully aware of the inherent risks and hazards there in by virtue of participating in the activities sponsored by the Somers Little League. Therefore, you on behalf of yourself and your children hereby elect to voluntarily engage in the activity despite said risks. In consideration thereof, Participant, or Participants parents being of lawful age does for himself/herself, and his/her heirs, executors, administrators and assigns, now releases and forever discharges, waives and covenant’s not to sue, any person or entity included but not limited to Somers Little League and all who or which succeed to Somers Little League’s interest, and it’s officers, directors, members, managers, coaches, agents, employees, independent contractors, or anyone else located at or related to the Activity or the Location, and each of them, all referred to herein as “RELEASEES,” for and on account of any and every claim, demand, action or right of action, of any kind of nature, either in law or in equity, known or unknown, from the Date hereof until the end of time. Participant agrees that this Assumption of Risk, Release, Waiver and Indemnity Agreement is intended to be as broad and inclusive as permitted by applicable laws of the state in which the venue/event is located and that if any portion of this Agreement is held invalid, it is agreed that the remainder of the agreement shall be in full force and effect.

Participant HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS THE RELEASED PARTIES and each of them from any loss, damage, attorney fees costs of litigation, or any other costs they may incur due to Participant’s misrepresentations herein and/or for any breach of this Agreement and the warranties contained here in including, but not limited to, any costs of litigation and attorney’s fees associated with any claim or suit related to Participant’s participation in the Activity at the Location.

PARTICIPANT UNERSTANDS THAT THIS IS A CONTINUING RELEASE AND INDEMNITY AGREEMENT WHICH NEVER EXPIRES AND APPLIES TO ALL INJURIES, DAMAGES, CLAIMS, LIABILITY, WHETHER ARISING THROUGH RELEASEES NEGLIGENCE OR OTHERWISE, AND/OR INHERENTLY DANGEROUS ACTIVITIES OCCURING PRIOR TO THE DATE OF ITS EXECUTION UNTIL THE END OF TIME.

I certify I am a legal United States citizen, or posses legal residency, or a visitor status to be in the United States, and that I will provide proof of said legal status if requested prior to or during any Somers Little League participation.

By signing below, you acknowledge that you (parent or legal guardian and player) have read and fully understood this waiver and all questions have been addressed. Participant further states that they have carefully read this Agreement and knowingly understands the content of this waiver and sign it of their own Free Will and intentionally waives any claims against the Somers Little League.
