Your trip is a significant investment, and we highly recommend the purchase of travel insurance. If you choose to decline this valuable protection, you are assuming any financial loss associated with your travel arrangements. By declining to purchase travel insurance you fully understand that your travel agent is not liable or responsible for any expenses incurred as a result of cancellation or interruption, emergency medical transportation, baggage and personal effects, flight and travel accident, and/or rental car physical damage.

Please read the travel insurance quote we provided carefully in order to make an informed decision concerning this important matter. Call the travel insurance plan provider with any questions you may have.

AT THE TIME OF DEPOSIT / FIRST PAYMENT:

I have been advised of the cancellation penalties for my purchase. I acknowledge receipt of the travel insurance quotation.

I understand that travel insurance can protect me from possible loss of money due to supplier bankruptcy/default, unexpected trip cancellation/interruption due to accident, sickness or death, baggage loss, medical expenses, and emergency air transportation costs.

I understand that I must purchase travel Insurance immediately to obtain maximum coverage.

**VERY IMPORTANT**

This contains your Terms & Conditions for your travel plans. Please review to ensure your preferred travel dates, room category, passport names, and birth dates are in order. Please sign the invoice (Only one time is required) & send it back to me within 7 days as the Travel Suppliers require your signature as legal acknowledgement.

Initial Deposit: The initial deposit to reserve your reservation is non-refundable.

CLIENT REQUESTS: Any and all requests made to the resort and transfer company are not guaranteed and Liz Moore Destination Weddings is not responsible for requests which are not granted by the resorts or transfer companies.

PASSPORT: Please ensure that your passport is valid for 6 months longer than your intended travel time. It is your responsibility to obtain at your expense all documentation required by all relevant government authorities for travel, which may vary depending on citizenship status. Special documentation may be required for unaccompanied minors or children traveling without both parents. You may be denied boarding the aircraft if you do not have the proper required documentation. Liz Moore Destination Weddings Ltd will not be held responsible for incorrect documentation whatsoever.

FLIGHT ITINERARY: Check with the airline at least 24 hours prior to each departure to ensure your flight is departing as scheduled. Airlines recommend Check-In 3 hours before flight departures. Passengers who do not allow sufficient time for all Check-In procedures as specified by the Airlines may be denied boarding and no compensation will be provided for missed Flights. Liz Moore Destination Weddings Ltd does not accept any liability for missed connections or for any expenses or for any expenses resulting from delayed, rescheduled, oversold, or cancelled flights.

BAGGAGE ALLOWANCE: Will vary & change frequently so you will need to contact the Airline you are traveling with directly to verify specific restrictions.

ENTRY & DEPARTURE TAXES: It is your responsibility to know and pay the appropriate amount of taxes to gain entry & departure of the countries you are planning on visiting. Most countries only take cash & typically USD.

GROUP BOOKINGS: If you are booked in a group and your group's size falls below the supplier's minimum requirements you agree to the possibility of a travel package increase.

DISCLAIMER Liz Moore Destination Weddings is acting as intermediary and agent for suppliers ("principals" identified on the attached or accompanying documents) in selling services, or in accepting reservations or bookings for services which are not directly supplied by this agency (such as cruises, air carriage, hotel accommodations, ground transportation, meals, tours, etc.). This agency, therefore, shall not be responsible for breach of contract or any intentional or careless actions or omissions on the part of such suppliers, which result in any loss, damage, delay, or injury to you or your travel companions or group members. Unless the term "guaranteed" is specifically stated in writing on your ticket, invoice or reservation itinerary, we do not guarantee any of such supplier's rates, bookings or reservation. Agents shall not be responsible for any injuries, damages, or losses caused to any traveller in connection with terrorist activities, social or labour unrest, mechanical or construction difficulties, diseases, local laws, climatic conditions, abnormal conditions or developments, or any other actions, omissions or conditions outside the agent's control. By embarking upon his/her travel, the traveller(s) voluntarily assume(s) all risks involved with such travel, whether expected or unexpected. The traveller is hereby warned of such risks and is advised to obtain appropriate insurance coverage against them. Your retention of tickets, reservations or bookings after issuance shall constitute consent to the above, and an agreement on your part to convey the contents hereto to your travel companions or group members.

** The client accepts full responsibility for any payments made towards their reservation.

I understand this is a disclosure of business practices and permissive payment agreement for the purchase of travel services. I agree that payments will be made directly to selected travel suppliers on my behalf by Liz Moore Destinations Wedding, agent Leah Burke. These payments will be made in prorated amounts not to exceed the total of approved charges for your travel. I understand Debit Cards often impose “Daily Charge Limits” and multiple payments may be necessary to pay the full balance due.

I am aware of any potential Cancellation, Refund and Surcharge Policies and Fees by suppliers and agree not to dispute or chargeback any of the above signed for and acknowledged charges without proper course of action.

Liz Moore Destination Weddings will not release personal information to ANY parties not listed on your finalized Travel Contract and will apply due diligence in maintaining records in a secure, safe location.

I hereby authorize Leah Burke, Liz Moore Destination Weddings, to charge the indicated credit card listed above for the travel services stated above. I guarantee and warrant that I am the legal cardholder for this credit card.
