VISIT CANCELLATION & NO SHOW POLICY
At Lifestyle Medical Centers, we schedule our appointments so that each patient receives the right amount of time necessary to address their needs and concerns with our Providers. It is very important to keep your scheduled appointment with us and arrive on time.

As a courtesy to help patients remember their scheduled appointments, Lifestyle Medical Centers sends text message and email reminders. Text reminders are sent out 3 days and 1 day prior, email reminders are sent out 3 days prior and telehealth appointments receive a second email approximately 45 minutes before the appointment.

If your schedule changes and you cannot keep your appointment, please contact us immediately so we may reschedule you and accommodate other patients waiting for an appointment. Please give us at least a 2-day notice. If you do not cancel or reschedule your appointment with at least a 2-day notice, we will assess a $60 “no-show / late cancellation” service charge to your account. This charge is not reimbursable by your insurance company. You will be billed directly for the fee and can reschedule your appointment once that fee has been paid. Likewise, if you are 15+ minute late to your scheduled appointment time, this will count as a no-show / late cancellation fee and a $60 fee will be assessed. After three consecutive no-show / late cancellations, our practice may decide to terminate its relationship with you.

If you need to cancel or reschedule a Consult or Restart visit within 2 days, we will require a $100 refundable deposit to be rescheduled for another time. The deposit will only be refundable if you show up for the rescheduled time and no further reschedules will be allowed.
