Absence Request/Excuse Form 2024-2025
This Absence Request/Excuse Form can be used for the following:
- As a parent note for illness or sickness (parents are allowed 2 parent notes per quarter). If your child is absent more than 3 days, a doctors note must be turned in to the appropriate division office to medically excuse the absence.
- Doctor or dentist appointments (medical notes should be turned in to the appropriate division office within 24 hours)
- For pre-approved requests (must be submitted at least 3 days prior to the absence and can be used for any reason). Students are allowed 4 days per year for pre-approved requests. For more information, refer to the Student Handbook.
- Per high school testing policy, no assessments may be taken early.
