    Terms of the Deposit:
    Your deposit is equal to four weeks of fees and is refunded to you at the end of the Fall Session or rolled over to the Winter/Spring Session. If you opt to reduce days before the end of the Fall Session, you forfeit the amount of the deposit equal to the reduction of days. (See Policies for more detailed explanation)

  • POLICIES

    Registration Terms:
    This form initiates your registration with Kids ‘N Dance’s Afterschool program for the 2021-2022 school year. Fall session begins at the start of school and ends at the beginning of winter break (per OUSD calendar). The second session extends from the end of winter break until the end of the year. The terms of this registration contract will remain in effect throughout both sessions unless written notice is given to discontinue attending for the second session.

    Deposit:
    The deposit, which is equal to one month’s fees, ensures your child’s space for one session and may be rolled over to a following session. It will be credited to the last month of your last session’s fees. At that time, any balance remaining will be refunded. In the first 2 weeks of November, we will ask that you to complete a “Change in Registration” to reduce days for the second session. Reducing enrollment prior to the end of the session or without submitting the “Change in Registration” form will result in a portion of the deposit being forfeited equal to the reduction of days per month. An increase in enrollment will require additional deposit equal to the number of days added per month.

    Calendar:
    Kids ‘N Dance’s after school program follows the Oakland Unified School District’s school calendar. You are only charged your registered days when OUSD schools are open. Additional care during professional development days and school holidays are not currently scheduled. Please discuss your needs with us as they come along and we will see if accommodations can be made.

    Registered Days:
    Each child is registered for specific days of the week. Days within one week may be traded as space allows and notice is given. Extra days needed beyond those registered for must be reserved in advance and will be added to the next month’s invoice. Your child’s current daily rate will apply. If your child is absent for a registered day, there are no make-ups or refunds.

    Kids ‘N Dance Classes:
    Classes are half off when taken on registered after school program days. Pre- registration discounts do not apply for classes that are already discounted. Musical theater parent opt out fees are not discounted.

    Sibling Discounts:
    Full payment is required for the sibling attending most often. 25% off will apply to siblings attending less often.

    Pre-Registration:
    Submitting this form with deposit will reserve your child’s spot for the fall session. Deposits are non-refundable if changes in registration are made prior to the end of the first session. Days of the week preferences are on a first come, first served basis.

    Payment:
    Fees are pre-paid by the first of the month. Invoices for the exact amount are emailed the week before. You are not charged for days OUSD is closed. Payments are considered late after the first Friday of the month. Afterwards, a $20 late payment fee/week applies.

    Check Processing Fee:
    Monthly payments by check have an added $10 surcharge. Having a credit card number on file can makes automatic payment possible. Invoices will be emailed to you monthly.

    Communication/Absences:
    If your child is absent for a registered day, there are no make-ups or refunds. Notify your child’s van driver of same day absences at least one hour before pick-up time to avoid confusion. A $5 fee will be charged if notification is not given. For advanced notice absences notify Marielle, not your child’s driver. Please program Marielle’s cell number in your phone (510) 703-7460 and your child’s driver’s number (to be provided) DO NOT use the general Kids ‘N Dance number urgent communication.

    Pick-up/Sign Out:
    Pick up from the program is 5:45 but will not be considered late until after 6:00. $1.00/minute will be charge for late pick up after 6:00. Please sign out your child daily.

  • POLICIES REGARDING REDUCING THE RISK OF COVID-19 TRANSMISSION

  • Children will be asked to wear a mask at all times except when eating socially distanced.
  • If any household member is ill with symptoms that could be attributed to Covid, we must be notified and your child must stay home. We will consult with Alameda County's Department of Health to determine criteria for your child to return to the program (negative Covid test, quarantine period, alternate reason for illness confirmed by doctor). Payment is still required for the time your child is absent. Refunds will only be provided if the facility closes temporarily due to a positive Covid case.
  • - I understand staff will sign my child out on my behalf.
    - I understand adults/family members are not permitted in the yard / facility
    - I understand that if my child shows any sign of sickness they will not be allowed to attend. And will need doctor's clearance or a negative covid test to return There is no credit for not attending.
    - Children will have to picked up immediately if they show signs of illness mid-day
    - Travel quarantine/testing protocols will have to be followed
    - I understand that if any family members refuse to adhere to our protocols after being reminded, they risk the child being dismissed from the program without advanced notice. Documentation of the reminder provided and policies not followed will be provided. No refunds will be provided if asked to leave the program
    - Children will be taught safety guidelines for participating in our program. We understand that they will not always remember but must always be willing to follow directions when reminded. If they seem to have difficulty with following directions and we feel it may potentially compromise the health of others in the program, the family will be notified and if not improved dismissal from the program is possible. Under these circumstances, refunds will not be provided.
  • Kids 'N Dance 'N Theater Arts ASP (Facility # 013421999) as a part of Onstage - Kids 'N Dance 'N Theater Arts, LLC has put in place preventative measures as recommended by the CDC, California and Alameda County Health Departments to reduce the spread of COVID-19. These measures are itemized in our Site Specific Covid Protection Plan and represents the policies and procedures we implement to help prevent the spread of Covid-19 to those in our daycare program and the family members that drop them off.

    However, Kids 'N Dance 'N Theater Arts ASP cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the Kids 'N Dance 'N Theater Arts ASP could increase your risk and your child(ren)’s risk of contracting COVID-19.

    By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending Kids 'N Dance 'N Theater Arts ASP and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Kids 'N Dance 'N Theater Arts ASP may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Kids 'N Dance 'N Theater Arts ASP
    employees, program participants and their families.

    I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at Kids 'N Dance 'N Theater Arts ASP.

    On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Kids 'N Dance 'N Theater Arts ASP, its employees, agents, and representatives, ofand from the Claims, including all liabilities, claims, actions, damages, costs orexpenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Kids 'N Dance 'N Theater Arts ASP,
    its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in Kids 'N Dance 'N Theater Arts ASP.

I understand that the on-line registration form with confirmation of emergency information and payment through September must be submitted ONE WEEK PRIOR TO THE FIRST DAY OF SCHOOL. This link will be provided in early August. If mid-year the registration link will be provided at the same time as this pre-registration AND MUST BE COMPLETED BEFORE FIRST DAY OF ATTENDANCE.
