WAIVER & PLAYER AGREEMENT (Below)
All Players must read and sign below to participate in the Combine. Even if you or your parent/guardian fail to sign below, your participation in the Combine signifies your and their agreement to all terms and conditions in the Waiver & Player Agreement below.

Player understands that she would not be permitted to participate in the Combine unless she and/or her Parent/Legal Guardian signs this Waiver and in exchange for being permitted to participate in the Combine agrees to the following terms and conditions:
PLAYER agrees that the Combine, Volley Hawaii, the Hawaii Volleyball Showcase, the Hawaii Volleyball Foundation and their sponsors may use pictures and/or videos taken of her at the combine for promotional purposes.
PLAYER acknowledges, and understands that (a) there are certain inherent risks of injury, dangers and hazards associated with participating in the activities at the Combine and that such risks may be significant, including the potential for serious injuries, permanent injuries and even death, including but not limited to bodily injury, concussions, strains, broken bones, fractures, viruses, ACL/MCL injuries, partial and/or total paralysis, death, or other injuries that could cause serious disability; (b) these risks and dangers may be caused by the negligence of the people running the event and/or persons assisting or participating in the same, the negligence of others, accidents, breaches of contract, the forces of nature or other causes; and (c) risks and dangers may arise from foreseeable or unforeseeable causes including but not limited to, balls striking PLAYER, equipment failing on PLAYER, PLAYER failing or bumping into others, equipment or facilities, PLAYER falling, running, jumping, landing or diving towards or away from a ball, consuming food or beverages at the event, decision making including how long sessions should run and such other risks, hazards and dangers that are integral to running such events.
Despite these inherent risks, PLAYER knowingly and voluntarily assumes all such risks, whether known and unknown, whether caused in whole or in part by the negligence of any entities or persons assisting, planning, instructing or working at the Combine and/or at any concession (collectively referred to as "the combine"), and assumes full responsibility for and/or relating to PLAYER's participation at the Combine. In exchange for PLAYER being allowed to participate in the Combine, PLAYER, on behalf of herself, her heirs, assigns, personal representatives, legal representatives and next of kin, hereby knowingly and voluntarily COMPLETELY RELEASES everyone associated in any way with the Hawaii Volleyball Combine including but not limited to The Hawaii Volleyball Foundation, The Hawaii Volleyball Combine, The Hawaii Volleyball Showcase, Volley Hawaii, participating Volleyball Clubs, Colleges, Universities, College Coaches, Combine coaches, USA Volleyball, the USAV Aloha Region, University Athlete, the State of Hawaii, City and County of Honolulu, and each of the aforementioned’s respective owners, officers, directors, members, managers, coaches, agents, employees, organizers, planners, agents and volunteers, from ANY AND ALL CLAIMS OR LAWSUITS OF ANY TYPE WHATSOEVER, for all types of damages to include but not limited to personal injuries, death, viruses, property damage, economic and/or educational opportunities and/or economical damage to PLAYER arising out of or relating to the Combine, and also agrees to defend and indemnify the said released parties from all claims or lawsuits that may be brought on her behalf relating to any injury, death or damage to PLAYER and/or her family members or friends, arising out of or relating to the Combine.
PLAYER understands and recognizes that while college teams have committed to the combine, their plans may change and as a result may not ultimately attend the combine.
Refunds shall be given to any PLAYER who is turned away from the combine. In all other instances, PLAYER understands that refunds shall only be given upon receipt of a written request with a physician's note certifying any injury on or before January 15, 2024. Refunds are not permitted for other reasons given that we order player uniforms ahead of time and incur substantial expenses.
PLAYER acknowledges and agrees that University Athlete or another comparable organization may receive their information in their registration for the purpose of creating player profiles for the college coaches.
VERY IMPORTANT REGARDING PAYMENT: When you submit this form, you will be directed to the Payments page. Payment MUST be made at the time of registration or your registration will not be accepted even though you will receive an automated email confirming you are registered.
