In consideration of this entrance registration for the Dance Starz Performance LLC, I waive all claims for myself, my heirs, any and all other owners and their representatives, successors, and assigns for any injury or illness which may result from my participation. I am in proper physical condition to dance in this intensive, by a licensed physician, as an amateur dancer. Further, I hereby give my permission to the Dance Starz Performance LLC and all studio owners to use my name and/or picture in any account of this event without limitation and obligation of further compensation.

Early Registration due by May 15th of $150 for discounted price, $200 after. All registration forms, waiver & money due by June 15th. Make checks payable to Dance Starz Perfromance LLC and mail all payments and forms to Shelby Smith at 703 Dana Drive Atco, NJ 08004. Once registration form and payment are received you will get a confirmation email holding your spot in the intensive.
