This form is for all donors who would like to have their giving statements sent via mail. You do not have to complete this form if you would like to receive your contribution statement via email. Please note that WORIC is not responsible for the time of delivery by USPS. Please allow 4-6 weeks for arrival due to the anticipated USPS delays.

*Please be advised if you are married, you and your spouse’s contributions will be listed together. If you desire to have separate giving statements, please email your request to finance@woric.org. Please include your name, address and phone number. This change will be effective for 2026 on the date the request is received by our WORIC Finance Department.

*If you desire to give through your business or organization for the upcoming 2026 calendar year, please email a request to finance@woric.org. Please include your name, your business name, your business address, and contact phone number.

*If any of your personal information has changed, please take this time to update your information using the online form: https://woric.org/membership-information-change-request. If you submit the request for an update, please be sure to update your personal profiles on the online giving method that you currently use.

I agree that the mailing address provided above is accurate, current, and the correct address to receive my statement by mail. I understand that by opting out of receiving an electronic version, it is possible that I will not receive my giving statement on or before January 31st due to the U.S. Mail System. I further acknowledge that if I have not received my statement by February 17, 2026, I will have to notify the WORIC Accounting Department via email at finance@woric.org on or after 2/17/2026. This request will remain in effect until canceled in writing.
