Bethel UMC Fall Fest - Vendor Application
EVENT: Bethel - Lewes Fall Festival
SPONSOR: Bethel United Methodist Church
DATE/TIME: Saturday. October 5, 2019. 9:00 am – 3:00 pm.
LOCATION: Bethel United Methodist Church. 129 W. Fourth St. Lewes, De. 19958
PURPOSE: ALL proceeds will benefit the Immanuel Shelter and other local charities.

ALL booth locations will be inside Bethel UMC.

FEE: $50 per booth space, which includes two tables and two chairs. Two vendors may share a space, one table each.

APPLICATION DEADLINE: September 27, 2019

Bethel UMC reserves the right to censor any vendor product or service not deemed appropriate for this venue.

Dear Vendor: Thank you for your interest in helping us support the critical needs of the Immanuel Shelter. To participate in this event, please complete the steps listed below.
(1) Provide the information requested on this form.
(2) Sign and submit this form.
(3) Using Bethel’s online payment option, pay the $50 fee per booth. (Link and directions will be provided after submitting this form)
(4) Print out a copy of the e-receipt.
