In response to COVID-19, CVS Health has partnered with the National Association of Free and Charitable Clinics (NAFC) to launch the Expanding Telehealth and Access to Healthy Food Grant for Free and Charitable Clinics and Pharmacies in California.
This $1M commitment to the NAFC will fund grants allowing NAFC member organizations in good standing to address the health care needs in the communities they serve and to provide expanded access to health care by implementing/expanding telehealth/telemedicine and strengthening food access.
$20,000, $25,000 or $30,000 grants will be awarded depending on the scope of the needs and related budget justification; partial grants may also be awarded. Grants will cover a period of 1 year.
Specific examples of funding under the Telehealth/Telemedicine category are as follows:
Please note - funding can be requested to implement a new telehealth/telemedicine program or expand an existing telehealth/telemedicine program.
Telehealth/Telemedicine
• Computers, tablets, laptops
• Telehealth/telemedicine system/software
• EMR implementation/Telehealth integration
• Training
• Cloud back-ups
• Internet services (cable and satellite)
• Staffing needs
Specific examples of funding under the Food Access/Food Insecurity category are as follows:
Please note- Funding can be used to support an existing Food Access Program only.
Food Access/Food Insecurity
• Purchasing Healthy Food
• Partnering with a Retailer to Provide Food Gift Cards
• Collaboration with a Food Bank partner
• Expansion of Food, Vegetable and Fruit Prescription Program
• A Dedicated Nutritionist or Dietician to work directly with patients
• Staffing needs
IMPORTANT NOTE: Up to 50% of the total grant request may be used for staffing, education or training and/or overhead (overhead includes rent, utilities, etc.).

Applications must be completed and submitted by 3:00pm Eastern Time (12:00pm Pacific Time) on June 19, 2020.

Amount and Use of Grant Funds:
$20,000, $25,000, $30,000 grants will be awarded depending on the scope of the project and related budget justification. The grants are available to California Free and Charitable Clinics, who are members in good standing of the NAFC. The award will cover a one-year period from May 2020-May 2021
Requested funds may only be used for the specific project request. Funding is not permitted to be reallocated or used for university or parent organization general or indirect overhead cost.
Grants cannot be used for the following:
• Debt retirement
• Costs already encumbered or to be reimbursed
• Lobbying and political activities
• Capital campaigns/improvements
• Evangelical work by religious organizations
• Individuals
• Fundraising drives/events
• Events/Conference production
• Endowment funds
• Capital improvements projects
• Consulting Fees
• Dental stand-alone projects
• Health Fairs
• Health Literacy stand-alone projects
• Meeting or Event Sponsorship
• Medication only requests
• Medical or office supply only requests
• Membership dues
• Mental health stand-alone projects
• Registration fees for meetings
• Needle Exchange Programs
• 100% funding to be used for salaries
Eligibility:
• The organization must be a member in good standing of the NAFC. If you are not a member of the NAFC, or are not sure, please email ariana@nafcclinics.org
• If the organization applying is a Free or Charitable Clinic, it must have a clinical team led by a Chief Medical Officer/Medical Director.
• The organization must designate a Project Director for this grant program.
• The organization must report results of the program at the 6 month mark and 12 month mark. The organization must provide success stories to the NAFC and CVS Health about the programs, patients served, caregivers and providers involved.
• The organization must utilize the funds for only the project that is approved; any use of funds on other projects or programs will immediately disqualify the Clinic from the rest of the grant period and all funds will need to be returned.
• The organization must not make any changes to the grant scope without written approval of the NAFC and the CVS Health.
• If the application includes a request for funds to hire a staff member, there must be a plan in place to have a staff member hired within the first three months of the grant award.
• Organizations who have received a CVS community award are eligible to apply for this grant.
• Organizations who were awarded the Coordinated Care for Improved Health Outcomes grant in the past are eligible to apply for this grant.
Important Notes:
• Applications will be accepted through this online grant application program through the form below.
• Applications will be scored on a 100-point system and the point values will be clearly defined in the applications.
• Faxed, emailed or mailed submissions WILL NOT be accepted.
• No brochures, attachments, videos or additional material will be accepted with the grant application.
• All sections must be answered in full and must follow the instructions or applications will be eliminated.
• The applicant organization will receive notification of NAFC’s receipt of the application; without receipt of the completed typed application, the grant will not be considered.
Reporting Requirements
All grantees will be asked to provide reporting on use of funds at the 6-month mark and at the 12-month mark of the grant period. Reporting will be submitted electronically using an online form provided directly to grantees by the NAFC.
