FBC Facility Reservation Form
Form should be completed in full and returned to Church for scheduling. Your event is not scheduled until confirmation is received form the Church Administrator and appropriate deposits (if any) are made. Please allow 5 work days for facility use decisions.

Security/Damage Deposit: A security/damage deposit equal to the facility use fee is required to reserve space. This fee is not applied to the facility use fee. It will be fully refunded provided facility is left as it is found.
