Allegations of non-sanctioned violations must be based on fact and may only be submitted to Iowa Soccer by a club official/staff member.
Prior to submitting the alleged violation, review the

If after reviewing the policy you believe there has been a violation, complete and submit this form. Your allegation must detail the alleged violation, section of the policy you believe to have been violated, and include documentation that supports your allegation.
All documentation and witnesses accounts supporting the alleged violation must be saved to a single word document or PDF in chronological order and submitted. If your allegation is submitted without supporting documentation, it will not be reviewed nor investigated.
