Please fill in the following questions for The Northwood Village Art, Dine and Design Musician and Entertainment Application / Information Request Form.

We are seeking local bands to showcase their talents at the Northwood Village Art, Dine and Design Event.

This form is what we will use to advertise you playing and also use the information for your payment.

Please choose dates that you would be interested in playing and we will contact you on what is still available.

The Northwood Village Art, Dine and Design is held every Last Friday of the month from 6 pm - 9 pm

Set up is from 5 pm - 6 pm please plan to arrive in enough time to start playing at 6 pm. Check in will be at the NW Corner of Northwood Road and Spruce Ave.

Note the Art, Dine and Design event is a rain or shine event. In the case of extreme weather, the event night could be canceled for the safety for all.

We provide a 15 ' x 15' tent, a 12' x 12' stage and electricity for this event.

We require you to email us an invoice for your services to HenEvent@gmail.com no later than 3 days prior to the event you would be scheduled for. (see details below)

I grant permission to all the foregoing entities and persons to use any photography, motion picture, recording or any other records of me in this event for legitimate purposes. I understand that all or part of the event can be canceled due to extreme weather.
