    Use this form to change the amount of a previously approved 2014-2015 Federal Direct Loan. This form is required to increase, decrease or cancel loans.

    To change the amount of a prior Direct Loan Request the Borrower must complete the appropriate sections below:

    *If you already received a refund for a loan, you will have to return funds before decreasing or cancelling your loan.

If I have requested a cancellation of a loan that has already credited to my account, I understand that I am responsible for paying the balance owed to Black Hawk College if a balance results from my request.
