Correctable Offense Form
You will use this form to submit proof of the correction for your "fix-it" offense. If you received more than one "fix-it" offense, you will need to submit one form per offense. Please call our office 812-244-2103 with questions. If there are issues with your submission, we will contact you by telephone, if possible, otherwise by US Postal Mail.

UTT or Case Number

Please enter the UTT number or Case Number. The UTT number is located in the top right of your printed ticket. It typically begins with three zeros (000) and contains only numbers. Your case number can be found at https://Mycase.In.Gov is begins with 84H01-####-IF-######.

A maximum of two photos per submission is allowed.
You may upload a photo or scanned image of the proof that you have corrected the offense. This could be a copy of your driver's license, vehicle registration or a photo of your vehicle's tail light.
You may take a photo of your receipt if you purchased a bulb, paid for a repair, etc. If you complete the field for UTT/Case Number, you do NOT need to submit a copy of your ticket.
If you are submitting proof of a headlight, brake light, tail light, turn signal, license plate light, etc. you will need to submit two photos. First photo will show the correction. The second photo will show your license plate. Obviously you cannot show a "blinking" turn light. So please submit a photo of the light as it is lit up.
