We are excited to invite you to showcase your amazing handmade crafts at our event!

This event is a wonderful opportunity for local artisans like you to share your unique creations with our vibrant community. There is no application fee, but items sold must be handmade or produced by the vendor.

PARTICIPATION AND PRODUCT RULES:
• Complete the registration form below and submit the online entry form before the deadline. Space is limited so reserve your spot before the deadline. If you have any questions or need further information, feel free to reach out to us at (650) 259-2360.

• Products should be suitable for all age groups and meet the following criteria: non-controversial content, age-appropriate design, safety compliance and positive themes.

• Product categories may include but are not limited to paintings, prints, jewelry, metalwork, mix media art, candles and home fragrances, fashion and accessories, sculptures, textiles and yarn crafts, glass art, up cycled and recycled art, stationery and paper goods, children’s toys and clothing, historical and cultural crafts, ceramics and pottery, bath and body products, leather goods, and home decor.

• Vendors are responsible to bring their own tables for their booths. We can provide the chairs. Please contact the Recreation Center at (650) 259-2360 and let us know what you need.

IN CONSIDERATION OF PERMISSION TO PARTICIPATE IN ANY CITY OF MILLBRAE (hereinafter referred to as "City") ACTIVITY OR PROGRAM,
AND/OR TO USE A CITY FACILITY, THE VOLUNTEER AGREES TO THE FOLLOWING:

1. VOLUNTEER VENDOR HEREBY RELEASES, WAIVES, DISCHARGES, AND HOLDS HARMLESS THE CITY OF MILLBRAE, ITS EMPLOYEES, OFFICERS
AND AGENTS (individually a "City Party," and collectively the "City Parties") from any and all claims against them for any loss, damage, or claim
therefore on account of injury to the undersigned, undersigned's family member, or property of the undersigned, including such claims or damages
incurred as a result of any negligent act or omission of a City Party. Volunteer also covenants not to sue the City Parties for any injury or claim resulting
from Volunteer's participation in a City activity or program, or use of a City facility or equipment, even if such claim or injury is a result of a City Party's
negligence or omission. It is further understood and agreed that this release, waiver, discharge, and covenant not to sue shall be binding on the
Volunteer's heirs and assigns.

2. VOLUNTEER VENDOR HEREBY ASSUMES THE RISK OF BODILY INJURY, DEATH OR PROPERTY DAMAGE while on City property, participating in a
City activity or program, or using a City facility and/or equipment, whether caused by any negligent act or omission of a City Party.

3. VOLUNTEER VENDOR EXPRESSLY AGREES that the foregoing release and waiver, indemnity agreement and assumption of risk, are intended to be as
broad and inclusive as permitted by California law and that if a Court determines any portion thereof is deemed to be held invalid said decision shall be
construed as narrowly as possible and the remaining clauses shall continue in full legal force and effect.

4. VOLUNTEER VENDOR HEREBY GRANTS CITY PERMISSION TO USE THEIR LIKENESS in any broadcast, telecast or print media free of charge. The City
may take and use photos/videos of Volunteer Vendor for publicity and marketing purposes. Photos/videos of Volunteer Vendors are used in the City’s Recreation
Guide, the City website, Social Media, and other City media publications.

VOLUNTEER VENDOR ACKNOWLEDGES THAT THEY HAVE READ THE FOREGOING AND ARE AWARE OF THE LEGAL CONSEQUENCES OF THIS
AGREEMENT, INCLUDING THE ACKNOWLEDGEMENT THAT IT PREVENTS THEM FROM SUING THE CITY OR ITS EMPLOYEES, AGENTS, OR
OFFICERS, FOR A PERSONAL INJURY OR INJURY TO PROPERTY THAT IS A RESULT OF VOLUNTEER VENDOR'S PARTICIPATION IN A CITY ACTIVITY
OR PROGRAM, OR USE OF A CITY FACILITY OR EQUIPMENT. VOLUNTEER UNDERSTANDS THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN VOLUNTEER VENDOR AND THE CITY OF MILLBRAE.

If Volunteer Vendor is under the age of 18, the Parent or Legal Guardian (If any) must sign below. By signing below, the minor Volunteer Vendor's Parent or Legal Guardian attests that the minor Volunteer Vendor has their permission to participate in the City program or activity, and/or use City equipment, and that all parties understand and agree to General Release and Waiver Agreement stated above.
