Have you tested positive for or do you suspect you may currently have COVID-19? IF SO, YOU MUST NOT ATTEND GYM 208 OR ANY ACTIVITIES AT THE CENTRE.
Have you developed any symptoms over the past couple of days typical of the Omicron variant - for example a sore or scratchy throat, headache or flu type symptoms?
Have you had any signs of a temperature higher than normal (ie over 37.5C in the past 2 days?
Have you experienced any new unexplained shortness of breath in the past 2 days?
Have you developed a new or continuous cough in the past few days?
Have you experienced any change to your sense of smell or taste the past few days?
Have you experienced any vomiting or diarrhoea over the past 2 days?

If you have answered YES to any of questions 2-7 above, you MUST NOT not attend the gym or any form of exercise or activity organised by Gym 208 or Glenariffe Community & Recreation Centre UNTIL you have carried out a Lateral Flow or PCR test which shows a negative result.
In submitting this form, you are confirming that the details above are true to the best of your knowledge and that you (or your guardian if you are under 18) understand and accept that you:

• Are participating at your own risk and accept the risks in attending.
• Undertake to wear footwear and clothing appropriate to the activity, prevailing weather conditions and location (ie whether indoors or outdoors).
• Should consult with your doctor before participating if you have any medical concerns.
• Assume full responsibility for any injuries, losses or damages that you may incur while undertaking either indoor or outdoor activity.
• Waive all claims against Friends of Glenariffe, Glenariffe Community & Recreation Centre, Gym 208 or Oisins CLG and/or our instructors/facilitators. *
