I am the parent and/or legal guardian with legal responsibility for the minor identified below. I understand that the ANA seeks to encourage its Young Numismatists to be socially engaged and give back to their communities through the ANA’s America 250: Connecting Coins With the Land(s) They Represent program. I understand that the ANA has not reviewed and has no control over the operations of the various volunteer opportunities which qualify for credit under the ANA’s aforementioned program.

I recognize that there are inherent risks associated with participation in any volunteer program which include, but are not limited to: 1) risk of injury from the volunteer activity and/or equipment utilized in the volunteer activity; and 2) risk of injury arising from employees, volunteers and other third parties associated with the organization for whom the volunteer services are provided.

I hereby release and hold harmless with respect to any and all injury, disability, damage, loss and death, whether caused by negligence or otherwise, the following persons or entities (herein referred to as “Releasees”): the American Numismatic Association, and its officers, directors, contractors, volunteers, employees, and agents. By executing this document, I agree to hold the Releasees harmless and indemnify them in conjunction with any injury, disability, damage, loss and death that may occur as a result of the minor identified below engaging in volunteer services.

I HAVE READ THIS RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, AND I FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT I HAVE GIVEN UP LEGAL RIGHTS FOR THE MINOR IDENTIFIED BELOW BY SIGNING IT. I REPRESENT THAT I AM LEGALLY AUTHORIZED AND LEGALLY COMPETENT TO EXECUTE THIS AGREEMENT, WHICH SHALL BE A BINDING COMMITMENT.
