Easter Flower Order Form
The Bethel Worship Committee is offering you again the choice of purchasing an Easter flower and/or a donation to the Cape Henlopen Food Basket in Memory/Honor of your loved ones. Please complete the following form by March 29 and then click SUBMIT at the bottom. Once submitted, you may complete your order by clicking the Bethel online payment link on the confirmation page.
If paying by check, please make your check payable to Bethel UMC (memo: easter flowers) and return it with this form to the church office at 129 W. Fourth St, Lewes, DE 19958 or the parsonage door slot no later than March 29.
If you'd like to pay online, simply complete this order form and click submit. You'll then be forwarded to the online portal where you can choose EASTER FLOWERS or CAPE HENLOPEN FOOD BASKET DONATIONS in the Fund Dropdown.
The flowers will be placed on the altar Sunday, April 4th. We request you leave the plants until after the 11:00 service that day.

Easter Lily (6” pot) - $8.00
Tulips (6" pot) - $8.00
Daffodils (6" pot) - $8.50

DEDICATION INFORMATION
Please provide information for one or more of the following options. Please read the choices carefully.
If you'd like to purchase a flower IN MEMORY OF a loved one, please enter the type of flower along with the name as you would like it to appear in the bulletin. Example: Tulip in memory of John Smith.
If you are purchasing more than one flower, list all of your memorials here (example: #1 - tulip in memory of Jane Green; #2 - daffodil in memory of John Smith)
If you'd like to purchase a flower IN HONOR OF a loved one, please indicate the type of flower and enter their name as you would like it to appear in the bulletin. Example: Tulip in honor of Jane Green.
