Use this form if you would like to enrol your child for the First Communion Programme being launched at the start of October 2023. If you do not have all the details, you can save the form and complete it later. (Just click "Save & Resume Later" at the bottom of the form, and make a note of the web address you are given to come back to it). Essential fields are marked with an asterisk*

Most children will be currently in Year 3 at school, but we can also consider applications for older children who have not yet received the Sacrament.

ALL CANDIDATES MUST HAVE BEEN BAPTISED IN A CATHOLIC CHURCH.

A separate programme is in place for children who have not been baptised or who were baptised in a non-Catholic church but wish to continue their faith-journey as Catholics.

Please read carefully the notes which accompany each of the questions, as these will give you a clearer picture of how the Programme will operate.

CLOSING DATE FOR APPLICATIONS: FRIDAY SEPTEMBER 29th 2023.

THE PROGRAMME WILL BEGIN AT ALL MASSES ON THE WEEKEND OF 30th SEPTEMBER/1st OCTOBER, and the children will be formally presented on the weekend of 4th/5th November.

  • We ask for your address so that we can confirm your residence within the parish, and in case we may need it for contact purposes in connection with the programme.
  • We need your email address in order to acknowledge receipt of your application, and to keep in touch as the programme progresses. (Please see note † below).
  • † As the programme progresses, we will need to contact you with information and updates and we normally do this by email - but in case we have difficulty getting in touch, we will use your phone number as a backup so that you do not miss out.
  • If you can give us a copy of your child's Baptism Certificate (or better still upload it below) this would be very helpful. If not, we will need to contact the church of baptism for verification. (If your child has not been baptised in a Catholic church, please use the form on the "Child/Young Person Reception" page instead).
  • If you have a copy of your child's Baptism Certificate which you could photograph or scan and upload here, it would save a great deal of time as we move forward. Thank you!

  • Most of the children on the Sacramental Programme will be attending Holy Cross and All Saints, St Gilbert's or St Mary's Primary Schools. We can also accept children from other schools, though normally we would expect them to be resident in this parish. Additional classes (normally on pre-arranged Saturdays) will be provided for those who attend non-Catholic schools, so that they do not miss out on those parts of the preparation which would otherwise be school-based.
  • Photographs are normally taken, and are displayed (without names) on a board in the churches. We need your permission for this, and will respect your wishes if you prefer your child's photo not to be taken.
  • Some of our Masses from Holy Cross are live-streamed on Facebook and YouTube, and recordings normally remain in place for later viewing. At some points during the programme we gather the children at the front of the church and, unless we take action to prevent it, they would be seen on camera. We will respect your wishes if you do not want your child to appear.
  • We are considering reintroducing Prayer Sponsors for each child - just the first name would be given for a parishioner to pray. More about this at the introductory meetings, but if you prefer your child's first name (unidentified) not to be shared we will, of course, respect this.
  • We will do everything we can to provide your child with the best Sacramental preparation we can. If you feel there is anything we need to know in order to serve your child's needs better, please tell us here and we will be in touch with you as necessary. Otherwise just leave this field blank.
  • TERMS

    The programme is all about completing your child's journey into the Church - the Family of God. Regular attendance at Mass on Saturday evening or Sunday is an ESSENTIAL PART of this journey.

    In submitting this form, you promise to help your child prepare for the Sacraments of Reconciliation and First Holy Communion, including the assurance of attendance at Mass, and agree to take part fully in the preparation provided by the parish. Members of the Sacramental Programme Team will assist you in every way they can.
