Before completing this application, please read the
Advisor Procedures. If you and your group can comply with the expectations of membership, please proceed by completing the below application and attaching four (4) teen reviews. The four (4) teen reviews must be each completed by a different teen and can be for any book that is published for young adults. Reviews must be submitted on the Teen Review Form.

Application Guidelines

1. All applicants must be current, personal members of YALSA at the time of applying.

2. All applicants must agree to a 2 year term and maintain membership in YALSA throughout their term.

3. Teen reviews must be attached in the indicated section on this application form as one document. (Please combine the reviews into one file, Word document or PDF preferred).

4. Applications are due no later than October 1, 2018
