State Delegate Trimester Report
Responses to the online form will be viewed by your District Director and Administrative Vice President by district due date. This form should be completed by the 5th of each month. Upon completion of the form, click Submit; you will receive a confirmation screen if your report has been successfully submitted.

You will need to send a separate email to your District Director and avp@mnwt.org of communication logs and any important back-up materials.
