Formal Student Complaint Policy Form
Use of the Formal Student Complaint Form:

LMU seeks to address written student complaints when brought to the attention of the administration. The Formal Student Complaint Process of LMU is a separate process from the Program Specific Grievance Appeal Process in an academic program. The University encourages students who have a legitimate concern to participate in the Formal Student Complaint Process if the concern is not addressed by the Program Specific Grievance Appeal Process. This form is used to document and track the institution’s forthright attempts to address appropriately filed Formal Student Complaints. The process initiated by this form does not negate or replace any Program Specific Grievance Appeal Process. The student may be directed to that process as a result of filing this form. The Program Specific Grievance Appeal Process must be completed by the student before any additional review may take place by the University. The Formal Student Complaint Process examines evidence of procedural violations of university policy in arriving at decisions rendered through the Program Specific Grievance Appeal Process. The review is not one of situation-specific evidence but only focuses on the University’s adherence to its stated policies and procedures. Upon receiving a decision through the Program Specific Grievance Appeal Process, students have thirty (30) days to begin the Formal Student Complaint Process. Students who wish to file a Formal Student Complaint must complete and submit this Formal Student Complaint Form. The Office of Institutional Compliance receives, processes, and archives Formal Student Complaints.

Students are required to use the program specific appeal/grievance process prior to filing a formal student complaint using this form.
