PLEASE NOTE: This trip is neither sponsored by, nor affiliated with, Liberty High School or Olentangy Local School District. It is an independent group of graduating senior friends and their families. In addition, all graduates (regardless if they are 18 years old) need to be traveling with a parent.

If you don't have a valid Passport Book at time of travel, you will be denied boarding at the airport (and trip funds will be forfeited to the supplier). The group's block of rooms is for standard view rooms- 2 double beds or 1 king. Please indicate your preference below. NOTE: If 4 people are traveling, it is a better value to get 2 rooms and have the additional space.

C. AIRFARE TO CANCUN- NOT INCLUDED
Package price of $825 per person (based on 2 per room) is for the land portion and DOES NOT include airfare. ALL PASSENGERS are responsible for getting their own airfare. The airport code is CUN. (Transfers are included between airport and resort, roundtrip.)

Delta and American Airlines often have the best schedules and rates. Southwest (not yet out for these dates) may also be a great option.

Air pricing and availability is always subject to change until booked by you. If you have questions, please let me know.

Powell Travel and Tours is happy to help guide you. But there is no group air contract, as it wasn't in the best interest of the group.

By leaving air OUT of the group pricing, you can purchase the schedule that works best for you. This way, you also have the opportunity to use airline points, companion pass, vouchers, etc. direct with the airline.

In order to coordinate the INCLUDED airport transfers, we will need flight info at a later date. YOU DO NOT have to be on the same flights as the whole group to quality for the transfers. I do recommend you travel on the same flights as others in your room.

E. TRIP INSURANCE
If you have to unexpectedly cancel your vacation, stiff penalties apply including full loss of vacation value. In addition, when traveling out the US, health insurance is not always valid. For these reasons, we recommend trip insurance.

The supplier has a policy, but Allianz is a preferred provider since they provide cash back (instead of travel vouchers) for covered reasons such as medical emergency, death in the family, and more.

F. RELEASE OF LIABILITY
On behalf of myself and my next of kin, heirs and representatives, I release from all liability Powell Travel Tours, LLC and its agents including Alison Turner, and vacation suppliers from any and all claims of physical or psychological injury, illness, death, damages, or economic or emotional loss (and my companions) may suffer because of my participation in this activity, including travel to, from and during the activity. NEXT STEPS
1. Upon receipt of this e-form, Powell Travel and Tours will create a booking for your party as part of the Liberty Group contracted block of rooms... at the Hard Rock Riviera Maya in a standard view room and make the deposit on your behalf.

NOTE: The deposit will not be automatic. This form collects the information for us to pay supplier. Payments will be processed around November 5.

2. If requested, Powell Travel and Tours will provide a no-obligation insurance quote from Allianz to you via email for you to accept or decline per timeframe stated in email

3. Final payment is due March 2, 2020. At that time, there will be an ABBREVIATED e-form for you to complete and submit final payment. I WILL NOT charge your same CC on file with out your authorization for final payment.
