We love how they included terms and conditions and authorization statements that make this a very effective release of liability form.

So, now we know…horses and dogs can seek physical therapy for injuries just like people! But, before patients can seek services from the Horse ‘N Hound Physical Therapy clinic, they must be referred by a veterinarian.

Fortunately, the clinic makes it a simple matter for vets to hop online, fill in the vet referral form online, and complete this step. This streamlined process ensures that their four-legged patients gain access to care sooner.

We love that this vet referral form includes a place where the vet must include a digital signature. Very nice!

And, for you cat lovers, no fear! There is an indication on the form that Horse ‘N Hound treats kitties with injuries, too!

Ard-Vista Animal Hospital: New patient intake form

Our quest for effective veterinarian forms led us next to the website of the Ard-Vista Animal Hospital and a comprehensive patient intake form.

They structured their form to be very easy for user’s to complete but still collect all the patient data they need to treat their new patient.

We like that they include their payment policy and ask pet owners to complete a checkbox agreeing to their terms.

Finally, we enjoyed how they put their logo on the form, reinforcing their brand.

Lone Peak Veterinary Hospital: Customer satisfaction survey

Lone Peak Veterinary Hospital uses several EmailMeForms on their website, making it easy for pet parents to have forms prepped before they arrive at the clinic. That must certainly take some of the stress off of the pets and their owners!

In particular, we loved their customer satisfaction survey. It’s no secret that we are EmailMeForm love feedback forms. If you’re a regular reader, you know we advocate that every company uses these forms. It’s exciting for us to see them embedded on a client’s website!

We noticed that this customer satisfaction survey includes a mix of checkbox statements, yes or no drop-downs, and open-ended questions. The form gives us the feeling that they care deeply about providing excellent service at this clinic.
